Your wellbeing survey

The Joined Up Care Derbyshire (JUCD) wellbeing service and local wellbeing teams deliver a comprehensive range of health and wellbeing programmes, activities, and initiatives to help colleagues remain healthy, safe and well at work.

To better understand your health and wellbeing needs, the network has developed your wellbeing survey - a chance for you to have your say and help us improve and develop more inclusive wellbeing support services.

We need your feedback by 31 July 2023 so we can continue to provide the best possible health and wellbeing support.

No identifiable information will be collected, meaning your responses are anonymous, and all data will be kept completely confidential.

Your Wellbeing Survey takes an average of 15 to 20 minutes to complete and as a thank you for taking part, you can enter a prize draw to win one of 2 wellbeing packs worth £195.
